How Operators Apply CRM

Anje and Tom are operators at Trident Minerals. They are dedicated to a fatality-free workplace and use the CRM process as the last line of defense against critical risks and hazards.

Pre-Start Meetings

They begin the day in their pre-start meeting. Here, Anje, Tom and their supervisor Jason plan to reinstall a repaired component.

Jason leads the task, working with the team to outline job steps and identify all potential risks. On the whiteboard, they use CRM's Critical Risk Icons to visualize and align each risk with its corresponding control.

IMPORTANT: These controls will need to be in place and fit-for-purpose before any installation work can begin.

Implement Controls

Anje and Tom implement their control measures:

  • A licensed rigger to secure the load.

  • Fall arrest devices and tool tethers.

  • An exclusion zone setup in the area below where they’ll be installing the component.

Perform a Verification

Anje and Tom then performed their verifications, ensuring all critical controls were in place and fit-for-purpose. During her verification, Anje discovered damaged links in the lifting equipment. She answers ‘No’ to this CRM checklist question

Stop Work Authority

They stopped the work and contacted Jason. He commended the team for their vigilance in finding a red and arranged a replacement from the tool store.

Although the lift was paused, Anje and Tom were able to continue with their site preparation activities until the replacement arrived, as the critical risk only related to the lifting operation.
